Normal slackware procedure to set up php is :

o Edit /etc/apache/httpd.conf and add index.php to the DirectoryIndex :
Code:
DirectoryIndex index.html index.php index.htm index.xhtml
o In /etc/apache/httpd.conf, uncomment this line :
Code:
Include /etc/apache/mod_php.conf
o Restart apache :
Code:
/etc/rc.d/rc.httpd restart
For mysql setup, read the comment in /etc/rc.d/rc.mysqld
