Here's an explanation from superuser.com by none other than Rod Smith, the rEFInd developer:

If a directory holds multiple kernels, rEFInd "folds" them into a single entry on its main menu by default. If you hit F2 or Insert, you'll see options for all the other kernels. (If refind_linux.conf has more than one line, each kernel will have multiple entries, one for each line in refind_linux.conf.)

If you prefer to see multiple entries on the rEFInd main menu, one for each kernel, you can achieve this result by editing the refind.conf file (usually in /boot/efi/EFI/refind or /boot/EFI/refind): Uncomment the fold_linux_kernels line and ensure the value is set to false.

Please notice that the two entries I listed are for 2 different installs, 2 different drives and partitions. My choices are substantially more complex than most because I boot so many different systems. Those are 2 0f 10 menued options. It is much like LILO but within that similarity exists how easy single option system can be. It is nice that no executable needs be run after editing refind.conf so it is trivial to add, alter or remove any trial attempts.. a useful learning tool I think.
