LAMP web poll software - suggestions?


You know those little polls that appear on websites such a slashdot, with a poll question, multiple choice answers, and a couple of buttons for "Vote" and "Results"?

Well, I'd like to put one on a website I'm developing, and I was wondering whether there is any open source web poll module (as it were) that I could drop into the home page and easily configure. The server runs LAMP, so a solution that uses MySQL and PHP would be ideal, but I'll consider others.

Any suggestions?
