Not receiving response from freeradius server on RHEL 5

I have freeradius 1.1.7 installed on RHEL 5 and radtest works with both local users and Active Directory. However, if I am not on the server running freeradius, and attempt to authenticate, the server does not respond in any way. Running freeradius in debug mode shows only the startup messages then "Ready to process requests". No indication that any request has come through. I have configured a Multitech MA820 modem bank and also my local Windows PC in the clients.conf file. Tried to connect via the modem and it times out ("Server did not respond in a timely manner"). Tried to connect from my PC using ntradtest and I get "Server not responding to requests". What am I missing?

can you run netstat on the server, and see is it listening for requests on any ports?

netstat shows this:

udp 0 0 *:radius-acct *:*

but the STATE is blank, it doesn't say LISTEN

Well, for some reason when you installed freeradius, it did not open the relevant port for you. Did you install from a reputable RPM base?

Found the problem - I was getting a dhcp address even though I though I had turned that off. I fixed in /etc/system/network-scripts/ifcfg-eth0 and now with the correct IP I am getting a response but it is Accept-Reject. The dialup error is "Access denied because the username and password was invalid on the domain." However, the same username/password works fine when I have the following line in my /etc/raddb/users file:

cyoho@umpublishing.org Auth-Type :=ntlm_auth

The error I see in the debug output is

rlm_realm: Looking up realm "umpublishing.org" for User-Name = "cyoho@umpublishing.org"
rlm_realm: No such realm "umpublishing.org"

but this is the realm defined in smb.conf, and it works when I do the test in the setup instructions (with the line cyoho@umpublishing.org Auth-Type := ntlm_auth in the users file,then radtest works)
